I have been a registered nurse for over 14 years and I am also a certified case manager. I most enjoy visiting clients in their own homes where I can truly get to know them and can devote my undivided attention to their individual care needs. I also enjoy educating and supporting caregivers, to assist them in providing the most dignified and compassionate care for their clients or loved ones. My nursing experience includes both in-patient and outpatient experience, in acute care, long-term care, and home health care. I worked for a Portland home health agency for six years as a home health case manager, visiting clients in their private homes, assisted living facilities or adult family homes. The majority of my clients were seniors. I also cared for home infusion and hospice clients. I am available to assist with care planning, medication management, home safety assessments, caregiver oversight, wound care and wellness checks. I am happy to assist with hiring caregivers, ordering equipment and getting my clients in touch with a variety of resources that would most benefit them. Most importantly, I will be a reliable advocate for my clients. If you are an out of town caregiver looking for an experienced professional to help manage your loved one's care, I can help. I can help bring clarity to any care giving situation and act as a liaison between multiple doctors and caregivers. Coordinating care is my specialty.
